Our applications for the upcoming school year 2024-2025 are now available. After submitting your application, you will be contacted to schedule a entrance exam for your child(ren) and answer any further questions you may have at that time.

We look forward for having you a part of our SPPS Panther family.

Age Requirements - The child must be on or before September 1st
Three (3) years old for Pre-Kindergarten (3)
Four (4) years old for Pre-Kindergarten (4)
Five (5) years old for Kindergarten
Six (6) years old for First Grade

How To Apply?

  3. Acceptance letters will be emailed 2 weeks after entrance exam
  1. Registration with the following required documents:
    • Original Birth Certificate
    • Baptismal Certificate (if applicable)
    • Communion Certificate (If applicable)
    • Form DH3040 School Entry Health Exam
    • Form 680 Immunization
    • Copy of last report card/transcripts
    • Copy of all standardized testing
    • Waiting list interview (if applicable)
    • Tuition Agreement
    • Home Language 

Registration fee to be paid through FACTS

Immunization Requirements

The Archdiocese of Miami requires that, prior to attendance in school, each student present or have on file with the school a certificate of immunization for the prevention of those communicable diseases for which immunization is required by the Department of Health. A completed Florida Certificate of Immunization (Form DH 680) is required to document the administration of prescribed immunization doses. An exemption from immunization requirements is permissible only with a physician’s certification as to the need for either a temporary or permanent medical exemption. An exemption from immunization requirements is not permissible for religious, philosophical, personal, or other reasons.
